Job summary

San Saba Rehabilitation and Nursing is seeking a dedicated Charge Nurse (PRN) who is an RN or LVN in good standing to lead care teams and ensure high-quality resident care in our San Saba, TX facility.

In this role you will administer medications, implement care plans, and document care while coordinating with shifts and supporting staff. This position emphasizes safety, efficiency, and compassionate patient outcomes in a dynamic nursing environment.

Qualifications

• Must hold RN or LVN license in good standing.
• Strong clinical, assessment, and documentation skills.
• Ability to communicate clearly and organize workflows.

Responsibilities

• Direct Care Teams: guide Nurse Aides to ensure resident care.
• Administer medications safely as prescribed.
• Implement care plans per policies and orders.
• Perform treatments as required for resident health and comfort.
• Transcribe physician orders accurately and promptly.
• Document resident care and complete nursing assessments.
• Participate in shift reports for continuity of care.
• Educate staff on nursing policies and resident needs.

We’re seeking a dedicated and reliable Charge Nurse – LVN or RN to join our growing team on a PRN basis! This role requires trustworthiness, consistency in attendance, and the ability to multitask effectively in a dynamic environment. A professional and cooperative demeanor is essential to collaborate with others and provide exceptional care to residents.

Your Impact as a (PRN) Charge Nurse – LVN or RN

In this role, you will:

• Direct Care Teams: Provide guidance to Nurse Aides to ensure appropriate care for residents.

• Administer Medications: Properly and safely administer resident medications as prescribed.

• Implement Care Plans: Execute resident care plans in compliance with company policies, federal guidelines, and physician orders.

• Perform Treatments: Deliver treatments as required to ensure resident health and comfort.

• Transcribe Orders: Accurately and promptly transcribe physician orders.

• Document Resident Care: Maintain timely and accurate documentation in resident charts and complete required nursing assessments.

• Collaborate During Shift Reports: Participate in shift reports to ensure seamless communication and care continuity.

• Educate Support Staff: Implement nursing policies and procedures, educating staff on the care needs of residents.

• Maintain Safety Standards: Promote a safe working environment by complying with infection control and body mechanic procedures.

• Address Staffing Needs: Respond to staffing shortages by arranging for replacement staff as necessary.
