Job summary

EVERGREEN CARE CENTER seeks an LVN/LPN to provide nursing care within scope of practice, ensuring patient needs are met per standards and orders. The role emphasizes accurate charting, patient condition documentation, and collaboration with physicians.

Responsibilities include following narcotic and restraint procedures, notifying departments of changes, and assisting with rounds as needed. A strong foundation in clinical care and willingness to pursue ongoing certification are valued.

Qualifications

• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

Responsibilities

• Follows established procedures for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents changes in patient conditions including nutrition, infections, skin conditions, weight loss, or other noticeable changes.
• Utilizes the Quality Life Manual for patient identification, placement, and monitoring in programs.
• Documents and notifies relevant departments of room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
