Signature HealthCARE in Johnson City, TN is seeking a dedicated LPN/LVN to provide direct nursing care, administer medications, and support residents with daily activities. You will supervise nursing assistants, ensure safety and comfort, and collaborate with the interdisciplinary team to meet individualized care plans.

The role requires an active LPN/LVN license, CPR/BLS certification, and at least one year of clinical nursing experience, preferably in long-term care, with strong documentation

Qualifikationen

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role.
• Strong clinical observation, medication administration, and documentation skills.
• Familiarity with electronic health records (EHR) systems.

Aufgaben

• Provide direct nursing care to residents, including medication administration and wound care.
• Supervise and direct nursing assistants and medication aides during shifts.
• Monitor residents' physical and emotional status and report changes to physicians.
• Develop, implement, and review individualized resident care plans with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy.
• Coordinate resident admissions, transfers, and discharges following procedures.
• Participate in quality assurance activities and regulatory surveys.
