Rio Grande Rehabilitation & Healthcare Center in La Jara, CO seeks a Charge Nurse (LPN or RN) to supervise nursing care, administer medications, and lead a nursing team on a full-time shift with rotating weekends.

You will monitor residents, document accurately, stay compliant with regulations, and support CNAs and other staff while coordinating with physicians and the interdisciplinary team to ensure high-quality resident care.

Formación

• Current Colorado LPN or RN license in good standing.
• Experience in skilled nursing or long-term care preferred.
• Strong leadership, communication, and critical thinking skills.

Responsabilidades

• Supervise and coordinate nursing care for residents during your shift.
• Administer medications and treatments as prescribed.
• Monitor residents' conditions and promptly communicate changes to physicians and the interdisciplinary team.
• Provide leadership, guidance, and support to CNAs and other nursing staff.
• Ensure accurate and timely documentation.
• Maintain compliance with all state and federal regulations and facility policies.
• Foster a positive, team-oriented environment focused on quality resident care.
