Job summary

Signature HealthCARE in Muncie, IN is seeking a caring LPN/LVN to provide direct nursing care, assist with ADLs, and supervise nursing staff on shift. You will support medication administration, wound care, and documentation while ensuring dignity, safety, and high-quality resident care.

The role requires an active LPN/LVN license in Indiana with LTC experience. Knowledge of EHR systems and MS Office is preferred; nurses may participate in QA and regulatory activities.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Familiarity with electronic health records (EHR) systems and MS Office.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with ADLs and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents’ physical and emotional status; notify physician of changes.
• Develop, implement, and review individualized resident care plans with the team.
• Complete accurate clinical documentation per policy and procedure.
• Coordinate resident admissions, transfers, and discharges.
• Participate in QA activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by Director of Nursing or designee.
