Job summary

Viridian Wellness and Rehabilitation Center is seeking a Charge Nurse to provide nursing care within scope, ensuring patient needs are met per standards and physician orders in a long-term care setting. This is a full-time role with a rotating schedule.

The position requires an active LVN/LPN license in Texas and BLS/CPR certification, with nursing management experience preferred. You will follow charting procedures, narcotics controls, and patient condition documentation, including physician

Qualifications

• Active LVN/LPN license in practicing state (Texas).
• Current BLS/CPR certification or willingness to obtain.
• Nursing management experience is preferred but not required.

Responsibilities

• Follows established procedures for charting and incident reporting.
• Follows procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents changes in patient conditions (nutrition, infections, weight, etc.).
• Utilizes Quality Life Manual for patient identification, placement, and monitoring in programs.
• Notifies departments of patient room changes, LOA, admits, or discharges.
• Makes rounds with physicians when necessary.

Your Job

The Charge Nurse will provide nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

Your Qualifications

• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

Your Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
