Job summary

Ivy Creek Wellness & Rehabilitation in Waco, Texas, is seeking a Charge Nurse for the 6A-2P shift. You will provide compassionate patient care and support day-to-day operations on the unit.

As a Charge Nurse, you will supervise staff, monitor patient conditions, document care, and ensure compliance with physician orders and center policies. A Texas LVN/LPN license and BLS/CPR certification are required or willing to obtain.

About the Role:

As a Charge Nurse on our 6A-2P team, you will be responsible for:

• Providing direct patient care and assisting with daily living activities
• Monitoring patient conditions and reporting any changes to physicians and other healthcare professionals
• Documenting patient information and updating medical records
• Collaborating with other healthcare team members to ensure seamless care coordination

Qualifications:

• Current LVN/LPN license in practicing state
• Current BLS/CPR certification or willingness to acquire one
• Experience in nursing management is preferred but not required

Responsibilities:

• Follows established procedures for charting and incident reporting
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction
• Documents any change in patient conditions
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs
• Documents and notifies applicable departments of patient room changes, LOA, new admits, or discharges
