CHI St. Vincent Hot Springs in Hot Springs, AR is seeking a RN Care Coordinator to guide patients through the healthcare journey, coordinating care across the continuum and advocating for optimal outcomes.

You will work with physicians, nursing teams, insurers, and post-acute providers to ensure high-quality transitions. The role emphasizes clinical acumen, communication, and strategic thinking, with leadership mentoring and staffing oversight as part of daily duties.

Qualifications

• Graduate of an accredited school of nursing and registered nurse in AR.
• Basic Life Support (BLS) required at hire; ACLS and PALS within 90 days.
• Preferred BSN enrollment or completion within 2 years.

Responsabilités

• Coordinate comprehensive care across the continuum and advocate for patients.

• Mentor and coach staff; assist with unit scheduling and staffing plans.

Job Summary and Responsibilities

As a RN Care Coordinator, you will be a central figure in patient care, seamlessly navigating the healthcare journey to achieve optimal outcomes and an exceptional patient experience.

Every day, you will strategically assess, plan, and facilitate comprehensive care across the continuum, expertly advocating for patients while collaborating with physicians, nursing, departments, insurers, and post-acute providers to ensure timely, high-quality transitions.

To be successful in this role, you will possess strong clinical acumen, exceptional communication and advocacy skills, and a strategic mindset, all driven by a passion for optimizing patient care across every touchpoint.

• HUMAN RESOURCE MANAGEMENT
• Functions as a clinical and leadership mentor and coach to new and existing staff
• Assists with preparation and oversight of unit schedules.
• Works with department management to balance schedules
• Oversees and adapts the daily staffing plan to meet the department's needs
• Establishes patient assignments for a team of employees.
