Responsibilities

• Provide wound care, catheter care, injections, and other nursing procedures as needed.
• Collect and monitor clinical information to support resident care plans.
• Recognize changes in resident conditions and communicate concerns promptly to appropriate team members.
• Assist with staff guidance, coaching, and support to promote quality care delivery.
• Collaborate with physicians, nursing leadership, residents, and families to ensure excellent care coordination.
