Elm Management in Lansdale, PA seeks a licensed practical nurse to deliver direct patient care under RN supervision. The LPN will serve as charge nurse on the unit and lead the CNA team through each shift.

The role involves med passes, wound care monitoring, glucose checks, EMR documentation, and shift-to-shift reporting. SNF/LTC experience is preferred, with new grads welcome to apply.

Formación

• Active PA LPN license required.
• BLS/CPR certification required.
• Experience in SNF/LTC preferred; new grads considered.
• Strong organization and ability to manage a full med pass.

Responsabilidades

• Administer medications and treatments accurately and on time.
• Monitor residents for changes and report to RN/physician.
• Carry out care plans and contribute to updates.
• Direct and support CNAs; ensure ADL care, turning, hydration.
• Document care, vitals, and meds in the EMR; report shifts.
• Process admissions, readmissions, and discharges as assigned.
• Follow infection control, safety, and resident rights policies.
