Job summary

Bristol Hospice is seeking a PRN Spiritual Care Clinician to support Brookfield, Greenfield & Menomonee Falls. You will perform spiritual assessments, help develop an individualized plan of care, and provide direct spiritual support to patients and families while collaborating with an interdisciplinary team.

Travel to hospice locations is required and flexibility with hours is essential; hospice experience is preferred.

Qualifications

  • Graduate of an accredited seminary or school of theology, BD, M.Div. or equivalent.
  • Experience working with death and dying; hospice experience preferred.
  • Working knowledge of Word, Excel, PowerPoint and EMR systems.
  • Willingness to travel to hospice locations and flexible hours.

Responsibilities

  • Assure spiritual assessment of patients and families/caregivers and timely services.
  • Facilitate the development of the individualized plan of care by participating in the assessment.
  • Provide direct spiritual care to patients and families/caregivers.
  • Serve as liaison to community chaplains and spiritual counselors.
  • Maintain records of spiritual care services for QA and policy review.
  • Attend patient care conferences as a member of the interdisciplinary team.
  • Recruit community chaplains and spiritual volunteers to meet needs.
