Job summary

TN Quality Care Hospice in Columbia, TN seeks a PRN Chaplain to provide spiritual care and bereavement support to patients and families. Flexible hours, with likely weekday shifts, and opportunities to work across an established hospice team.

The role emphasizes interfaith sensitivity, compassionate service, and collaboration with the interdisciplinary team. A seminary degree and ordained status are preferred; valid driver’s license required.

Qualifications

• Graduate of an accredited seminary or school of theology.
• Certificate of ordination or commission from a religious faith group; completion of Clinical Pastoral Education (CPE) preferred.
• Previous hospice experience or comfort with interfaith settings is advantageous.

Responsibilities

• Performs spiritual bereavement assessments of patients and families in the hospice program and ensures identified needs are addressed in the plan of care.
• Provides direct spiritual care and bereavement counseling to patients and families/caregivers.
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers; assists team members as needed to ensure service quality.
• Provides accurate and timely documentation of patient services reflecting the plan of care.
• Attends group meetings to coordinate care and support interdisciplinary collaboration.
• Offers consultation, education, and support to the interdisciplinary group on spiritual care and bereavement counseling.
• Participates in Continuous Care and on-call responsibilities as needed.
• Assists in supervising spiritual care volunteers for patients' families/caregivers.
• Directs development and implementation of the bereavement plan of care and maintains bereavement services for up to one year after the patient's death.

Qualifications:

• Provides direct spiritual care and bereavement counseling to patients and families/caregivers
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assists team members as needed to ensure that this level of service is consistently provided
• Provides accurate and timely documentation of patient services to reflect the plan of care
• Attends group meetings to facilitate coordination of care and discussion of interdisciplinary group involvement
• Provides consultation, education, and support to the interdisciplinary group on spiritual care and bereavement counseling
• Participates in Continuous Care and on call, if necessary
• Assists in supervision of spiritual care volunteers when assigned to patient’s families/caregivers as needed
• Assumes responsibility in directing the development and implementation of the bereavement plan of care and the frequency of service delivery; make available bereavement services for up to one year following the death of the patient
