Job summary

TN Quality Care Hospice in Columbia, TN is seeking a PRN Chaplain to provide spiritual care to patients and families in our hospice program. This role offers flexible hours and a supportive, patient-centered team.

You will conduct bereavement assessments, deliver counseling, coordinate with the interdisciplinary group, and participate in on-call coverage as needed. A seminary degree and interfaith sensitivity are essential.

Qualifications

• Graduate of an accredited seminary or school of theology; ordination or commission preferred.
• Hospice experience with death and dying strongly preferred.
• Comfortable in an interfaith setting and respectful of diverse beliefs.
• Valid driver’s license and proof of insurance required.

Responsibilities

• Performs spiritual bereavement assessments and integrates needs into the plan of care.
• Provides direct spiritual care and bereavement counseling to patients and families.
• Demonstrates excellent service to patients, families, referral sources, and staff; supports team.
• Documents patient services accurately and timely to reflect the plan of care.
• Attends group meetings to coordinate care and discuss interdisciplinary involvement.
• Provides consultation and education on spiritual care to the interdisciplinary group.
• Participates in Continuous Care and on-call duties as needed.
• Supervises spiritual care volunteers as assigned.
• Directs the bereavement plan of care and offers bereavement services for up to one year after death.

Qualifications

• Graduate of an accredited seminary or school of theology and/or a certificate of ordination or commission from a religious faith group; completion of Clinical Pastoral Education (CPE) preferred
• Previous hospice experience working with death and dying strongly preferred
• Comfortable in an interfaith setting and able to accept different lifestyles, cultures, beliefs, and values
• Valid driver’s license and proof of insurance is required

What You’ll Do

• Performs spiritual bereavement assessments of patients and families/caregivers in the hospice program in a timely manner and ensure that identified needs are addressed in the patient’s plan of care
• Provides direct spiritual care and bereavement counseling to patients and families/caregivers
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assists team members as needed to ensure that this level of service is consistently provided
• Provides accurate and timely documentation of patient services to reflect the plan of care
• Attends group meetings to facilitate coordination of care and discussion of interdisciplinary group involvement
• Provides consultation, education, and support to the interdisciplinary group on spiritual care and bereavement counseling
• Participates in Continuous Care and on call, if necessary
• Assists in supervision of spiritual care volunteers when assigned to patient’s families/caregivers as needed
• Assumes responsibility in directing the development and implementation of the bereavement plan of care and the frequency of service delivery; make available bereavement services for up to one year following the death of the patient
