Job summary

Job Description

To provide spiritual and emotional care to chronically and terminally ill patients using the Palliative Care Transdisciplinary Model. The chaplain gathers and evaluates data pertinent to the patient’s situation including psycho‑social‑spiritual/religious health in collaboration with a transdisciplinary team of doctors, nurses, social workers, pharmacists, patients, families, and caregivers. The chaplain develops and implements a plan of care to promote patient well‑being, continuity of care, quality of life, and end‑of‑life goals. The chaplain models and collaborates within the organization and its disciplinary team to provide competent, patient‑ and family‑centered care respecting the cultural diversity of the population served. The chaplain facilitates the completion of advance directives (living will and power of attorney for health care) and provides information to patients and families regarding community resources.

Responsibilities

Provide pastoral care to referred patients/families and to staff/physicians.

  • Assess for anticipatory or complicated grief.
  • Provide in‑person and tele‑chaplaincy spiritual care to patients/families.
  • Provide bereavement care to families.

Collaborate with the palliative care team, including:

  • Providing spiritual assessment for each referred patient and family.
  • Facilitating completion of advance directives (living will and power of attorney for health care).
  • Attending and contributing to team rounding and conferencing.
  • Assisting in educational presentations to patients, families, staff, physicians, and community care providers.

Assist other bereavement support resources and spiritual care providers, such as community religious and spiritual care ministers, community‑referred agencies, etc., in maintaining a high level of continuity of care.

Qualifications

Education: Bachelor’s required, master’s preferred, four clinical pastoral education units, palliative care continuing education.

Years of Experience: Five years of experience in health‑care chaplaincy preferred.

Required Skills and Knowledge: Clinical pastoral skills with emphasis in palliative and bereavement care.

Licenses & Certifications:

  • Official endorsement, licensure, and/or ordination in a religious organization.
  • Board certification for Chaplains from the Association of Professional Chaplains preferred.
