Northwestern Medicine is seeking a Chaplain for Home Health Hospice. This role provides spiritual counseling to patients, families, and staff, and aligns with NM's mission and compliance standards.
You will assess spiritual needs, perform home visits, and collaborate with the hospice team. The ideal candidate has formal training in pastoral counseling, at least five years of experience in health care settings, ACPE CPE, and a master's degree in theology preferred.
The Chaplain - Home Health Hospice reflects the mission, vision, and values of NM, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and all other regulatory and accreditation standards.
Provides or arranges for spiritual counseling and pastoral care to patients, families and staff. Assesses, treats and cares for patients' spiritual needs demonstrating an understanding of the cognitive, physical, emotional, and chronological maturation process. Responds to patients while demonstrating an understanding of the Hospice concept, policies and procedures.
A religious professional who has a combination of documented formal training in pastoral counseling and supervised counseling. Experience in a health care or clinical setting. Academic and supervised work experience must equal at least five years. Previous experience in Hospice or Home Care preferred.
