Job summary

The Care Team Hospice in Decatur, IL is seeking a compassionate Hospice Chaplain to provide spiritual care and emotional support to patients and families at the end of life. You will work with an interdisciplinary team to assess needs, plan care, and document interventions with sensitivity to beliefs and traditions.

This role is field-based, offering meaningful bedside support, bereavement services, and collaboration with community faith leaders.

Qualifications

• Ordained, commissioned, or in good standing with a denomination.
• Bachelor's degree preferred; M.Div. preferred.
• Experience in spiritual care within hospice, healthcare, or pastoral settings.
• Knowledge of hospice philosophy, grief support, and end-of-life spiritual care.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide compassionate spiritual care and emotional support tailored to beliefs and preferences.
• Complete spiritual assessments, develop care plans, and document in the EMR.
• Collaborate with the interdisciplinary team on care planning and bereavement services.
• Connect patients and families with faith leaders and spiritual resources as needed.
• Deliver ongoing bereavement support and education to families.

The Care Team Hospice is seeking a compassionate and empathetic Chaplain to provide spiritual and emotional support to patients and families during life's most meaningful moments. As a Hospice Chaplain, you'll partner with our interdisciplinary team to provide individualized spiritual care, emotional guidance, and bereavement support while honoring each patient's beliefs, values, and traditions. Qualifications: Key Responsibilities

• Provide compassionate spiritual care, counseling, and emotional support to hospice patients and their families based on individual beliefs, values, and preferences.
• Complete spiritual assessments, develop appropriate care plans, and document interventions in the electronic medical record.
• Collaborate with the interdisciplinary team to support patient care planning, symptom management, and bereavement services.
• Connect patients and families with community faith leaders and spiritual resources when requested or appropriate.
• Provide ongoing bereavement support and education to families while promoting dignity, comfort, and quality of life throughout the hospice journey.

Qualifications

• Ordained, commissioned, or otherwise recognized and in good standing with a religious denomination or faith tradition.
• Bachelor's degree preferred; Master of Divinity (M.Div.) or equivalent theological education preferred.
• Previous experience providing spiritual care; hospice, healthcare, hospital, or pastoral care experience preferred.
• Knowledge of hospice philosophy, grief support, and end-of-life spiritual care.
