Job summary

Ambercare Hospice in Las Cruces, NM seeks a compassionate Chaplain to join our interdisciplinary team. You will provide spiritual care, bereavement support, and compassionate presence to patients and families at end of life.

You'll collaborate with clinicians to address spiritual and emotional needs, participate in care planning, and offer guidance that honors diverse faiths and beliefs. The role offers meaningful patient connections and opportunities for professional growth within Addus

Qualifications

  • Graduate of an accredited seminary or school of theology; ordination or commission preferred.
  • Hospice experience and bereavement counseling experience are advantageous.
  • Comfortable in an interfaith setting and able to respect diverse beliefs and lifestyles.
  • Valid drivers license and proof of insurance required.

Responsibilities

  • Performs spiritual care and bereavement assessments for patients and families.
  • Provides direct spiritual care and counseling to patients and caregivers.
  • Documents services accurately in the plan of care.
  • Collaborates with the interdisciplinary team to coordinate care.
  • Supports on-call coverage and supervises volunteers when assigned.

What Youll Do
  • Performs spiritual bereavement assessments of patients and familiescaregivers in the hospice program in a timely manner and ensure that identified needs are addressed in the patients plan of care
  • Provides direct spiritual care and bereavement counseling to patients and familiescaregivers
  • Demonstrates exceptional service to patients families referral sources co workers and other customers
  • Assists team members as needed to ensure that this level of service is consistently provided
  • Provides accurate and timely documentation of patient services to reflect the plan of care
  • Attends group meetings to facilitate coordination of care and discussion of interdisciplinary group involvement
  • Provides consultation education and support to the interdisciplinary group on spiritual care and bereavement counseling
  • Participates in Continuous Care and on call if necessary
  • Assists in supervision of spiritual care volunteers when assigned to patients familiescaregivers as needed
  • Assumes responsibility in directing the development and implementation of the bereavement plan of care and the frequency of service delivery; make available bereavement services for up to one year following the death of the patient
Qualifications
  • Graduate of an accredited seminary or school of theology andor a certificate of ordination or commission from a religious faith group; completion of Clinical Pastoral Education CPE preferred
  • Previous hospice experience working with death and dying strongly preferred
  • Comfortable in an interfaith setting and able to accept different lifestyles cultures beliefs and values
  • Valid drivers license and proof of insurance is requiredACHOS
