Job summary

Serenity Hospice in Portland Metro is seeking a PRN Chaplain to provide spiritual care and bereavement support on a as-needed basis. Schedule is as needed (0-30 hours/week) and on-site to support patients, families, and the care team.

You will perform spiritual bereavement assessments, deliver direct spiritual care, document services, and collaborate with interdisciplinary teams. The role may include on-call duties and supervising volunteers as needed.

Qualifications

  • Graduate of an accredited seminary or school of theology and/or ordination or commission from a religious faith group; CPE preferred.
  • Hospice experience with death and dying strongly preferred.
  • Interfaith setting comfort; acceptance of diverse beliefs and cultures.
  • Valid driver’s license and proof of insurance required.

Responsibilities

  • Performs spiritual bereavement assessments for patients, families, and caregivers.
  • Provides direct spiritual care and bereavement counseling.
  • Delivers exceptional service to patients, families, and care teams.
  • Documents services accurately and promptly reflecting the plan of care.
  • Attends group meetings to coordinate care and interdisciplinary collaboration.
  • Offers consultation and education on spiritual care and bereavement to the team.
  • Participates in Continuous Care and on call, if needed.
  • Supervises spiritual care volunteers when assigned.
  • Directs bereavement plan of care and ensures services for up to one year after death.

What You’ll Do
  • Performs spiritual bereavement assessments of patients and families/caregivers in the hospice program in a timely manner and ensure that identified needs are addressed in the patient’s plan of care
  • Provides direct spiritual care and bereavement counseling to patients and families/caregivers
  • Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assists team members as needed to ensure that this level of service is consistently provided
  • Provides accurate and timely documentation of patient services to reflect the plan of care
  • Attends group meetings to facilitate coordination of care and discussion of interdisciplinary group involvement
  • Provides consultation, education, and support to the interdisciplinary group on spiritual care and bereavement counseling
  • Participates in Continuous Care and on call, if necessary
  • Assists in supervision of spiritual care volunteers when assigned to patient’s families/caregivers as needed
  • Assumes responsibility in directing the development and implementation of the bereavement plan of care and the frequency of service delivery; make available bereavement services for up to one year following the death of the patient
Qualifications
  • Graduate of an accredited seminary or school of theology and/or a certificate of ordination or commission from a religious faith group; completion of Clinical Pastoral Education (CPE) preferred
  • Previous hospice experience working with death and dying strongly preferred
  • Comfortable in an interfaith setting and able to accept different lifestyles, cultures, beliefs, and values
  • Valid driver’s license and proof of insurance is required
