Job summary

St. Croix Hospice seeks a Chaplain to provide emotional, practical and spiritual guidance to patients and families on their hospice journey, coordinating with the clinical team.

The Chaplain will conduct spiritual assessments, develop care plans, lead bereavement activities, and oversee volunteers and community counselors, while ensuring HIPAA compliance. Qualifications include a Bachelor's degree (or 5 years ministerial experience), theological coursework, valid driver’s license, and preferred

Qualifications

• Bachelor's degree required; in lieu of a degree, a minimum of 5 years of experience in a formal ministerial setting is required.
• Theological coursework required.
• Valid State Driver’s License, access to an automobile, and acceptable driving record required.
• Preferred: MDIV, MTS, ThM or equivalent theological degree; one or more units of Clinical Pastoral Education preferred.

Responsibilities

• Conduct spiritual assessments and develop individualized care plans.
• Provide direct spiritual support to patients and families.
• Coordinates with community chaplains and spiritual counselors.
• Maintains records for quality assurance and compliance.
• Attends interdisciplinary team meetings and provides spiritual care consultation.
• Recruits and educates community chaplains, counselors, and volunteers.
• Assists in developing and maintaining a bereavement program, ensuring follow-up care.
• Oversees condolence calls and support group scheduling.
• Conducts funeral/memorial services as requested.
• Organizes periodic memorial services for staff, volunteers, and clergy.
• Supervises spiritual care volunteers as needed.
• Participate in quality assessment and performance improvement activities.
