Job summary

Crest Home Health & Hospice seeks a compassionate Chaplain/Spiritual Counselor to provide spiritual and emotional support to hospice patients and their families in a home-based setting. The role requires sensitivity to diverse beliefs and strong interpersonal skills.

You will assess spiritual needs, participate in IDG meetings, and coordinate with clergy and community resources to address bereavement and memorial needs. A seminary degree and pastoral ministry certification are preferred.

Qualifications

• Graduate from an accredited seminary or school of theology.
• Certification or degree in pastoral ministry is required/preferred.
• Two years of chaplain/spiritual counselor experience preferred.
• Direct patient or client contact experience preferred.
• Ability to work independently in a home-based hospice setting.
• Strong interpersonal and communication skills with diverse groups.
• Valid driver’s license, auto liability insurance, and reliable transportation.

Responsibilities

• Provide spiritual and emotional support to hospice patients and families based on beliefs and preferences.
• Assess spiritual needs and participate in interdisciplinary group (IDG) meetings to coordinate care.
• Support patients and families during end-of-life care, at death, and through bereavement.
• Coordinate with clergy, faith communities, and community resources for additional support.
• Provide or assist with memorial and funeral services when appropriate.
• Maintain accurate documentation of visits and services.
• Educate hospice staff and community on spiritual needs related to illness, loss, and grief.
• Respond to urgent or crisis situations with appropriate support.
