Position Title: CFSP Foster Care / Adoption Care Manager (Managed Care Coordinator)

Job Description:

Managed Care Coordinator
Sign-on Bonus: $1000

Location: Field: This role involves traveling to client sites or designated locations, with occasional office attendance for meetings or training. It requires interaction with patients, members, or providers in person four to five days per week.

We are partnering with North Carolina DHHS to implement a statewide Medicaid Plan supporting Medicaid-enrolled children and families in the child welfare system, ensuring access to comprehensive health services and coordinated care.

The CFSP Foster Care/Adoption Care Manager manages individual service plans within the NC CFSP Program, in accordance with state law and contracts.

How you will make an impact:

  • Provide integrated, whole-person Care Management across physical health, behavioral health, I/DD, LTSS, pharmacy, and unmet needs.
  • Offer Trauma-Informed Care, recognizing ACEs and coordinating cross-agency support for diverse needs.
  • Collaborate with County Child Welfare Workers to align healthcare with permanency goals.
  • Coordinate care with multidisciplinary teams, including health, behavioral health, and child welfare professionals.
  • Conduct assessments to identify and evaluate member needs.
  • Identify members with clinical healthcare needs and coordinate with healthcare management teams.
  • Oversee non-clinical needs of members with chronic conditions for efficient service use.
  • Set goals collaboratively with members, caregivers, and providers.
  • Identify members who may benefit from expanded services.

Minimum Requirements:

  • BA/BS degree and at least 1 year of experience working directly with the program population or related community organizations, or equivalent education and experience.

Preferred Skills, Capabilities, and Experiences:

  • Reside in North Carolina.
  • Degree in health, psychology, sociology, social work, nursing, or related fields preferred.
  • Two years of experience working with child welfare system clients preferred.
  • Willingness to travel as needed.
