FMRC Health Group is seeking an in-home case manager to support adults with developmental, cognitive, or physical disabilities in California. This role emphasizes Spanish-speaking families and requires compassionate, community-based outreach.
You will coordinate services, meet clients where they live, assist with navigating benefits and providers, document progress, and communicate with families and care teams to promote independence and well-being.
Support adults with developmental, cognitive, or physical disabilities as they build independence at home and in their community. Spanish speaking families are counting on this role.
Responsibilities:
Coordinate services and practical support for each client
Meet clients in their homes and community settings
Help clients and families navigate benefits, providers, and appointments
Document progress and communicate with families and care teams
Qualifications:
Associate or bachelor degree in human or social services plus one year working with adults with developmental, cognitive, or physical disabilities
Three or more years of adult case management may qualify in place of the degree and one year requirement
Fluent Spanish
Valid driver license, reliable vehicle and insurance
