Job summary

CCARC, Inc. in Berlin, CT seeks an Employment Program Coordinator to oversee the coordination, direction and supervision of the Employment program, delivering person-centered services and organizing IP meetings.

The role includes intake, orientation and discharge processes, and coordinating data-driven assessment and documentation. Ideal candidates will have supervisory experience, strong communication and negotiation skills, plus experience in developing service plans and community-based

Qualifications

  • Must have High School Diploma or GED with 3+ years in disabilities field; supervisory experience preferred.
  • Bachelor’s in related field with 1+ year in disabilities required; 3 years for HS/GED option.
  • Customized Employment Certified is required.
  • CPR, First Aid, PMT and DDS Medication Administration certifications needed.

Responsibilities

  • Coordinate, direct and supervise the Employment program.

Salary Range: $53,500.00 To $55,000.00 Annually

Hours

Monday - Friday, 35 hours/week. Flexibility may be required based on the needs of the program.

Primary Purpose

The person in this position will be responsible for the overall coordination, direction and supervision of their assigned Employment program, utilizing a person-centered approach to delivery of services. This will include, but is not limited to, facilitation of community based inclusive opportunities, Completing assessments for individuals in the Employment program, supervision of staff and case coordination. This position will function according to the policies established by the CCARC, Inc. Board of Directors, with the goal to maximize the potential of the individuals in the program.

Essential Functions
  • Treat all individuals with respect and dignity, encourage opportunities that promote choices and promote incidental teaching in the service areas
  • Hire, train, direct, supervise and evaluate assigned staff.
  • Develop, implement and monitor a comprehensive and diversified day services to meet the interests and needs of individuals, in accordance with the Agency’s Mission.
  • Responsible for intake, orientation and discharge process for individuals referred to or receiving services from CCARC’s Day Services.
  • Organize, schedule and facilitate Individual Plan (IP) meetings with individuals receiving services.
  • Assist and monitor in the development and implementation of service plans, assessment reports, Annual reviews, bi-annual reviews, quarterly reviews as needed and data collection, behavioral programs and other pertinent records.

  • Develop and maintain a cooperative relationship with a variety of community organizations, community venues and businesses, in order to provide meaningful opportunities for inclusion and/or job placements.
  • Assure that the Agency, Department of Development Services, Department of Rehabilitation Services and Department of Labor and any other contracting agency’s policies and procedures are followed
  • Monitor and remain knowledgeable of the operating budget of the services area. .
  • Review, process and maintain all required documentation, data and records relevant to appropriate Day Service area
Other Related Responsibilities
  • Attend all agency staff meetings or other meetings.
  • Requisition supplies and equipment in accordance with Administrative Policies.
  • Keep informed and implements current trends and methods.
  • Maintain good daily attendance (days of absence, illness).
  • Work with fellow staff cooperatively and effectively; ensuring flexibility and coordination of services.
  • Ability to work responsibly when working alone.
  • Ensure the highest standards of health, sanitation and safety.
  • Assist in arranging transportation for individuals

Education and/or Work Experience Requirements
  • High School Diploma / GED
  • For a Bachelor’s Degree in a related field, minimum experience required would be 1 year in the field of disabilities. For High School/GED, minimum experience required would be 3 years in the field of disabilities. Supervisory experience preferred.
  • Customized Employment Certified.
  • Must have valid Driver's License, be insurable with Agency carrier. Individual Home Support employees are required to have a vehicle for use and maintain adequate insurance.
  • Must complete the Agency’s Employee Training
  • Certification and training, including but not limited to: CPR, First Aid, PMT (Psychological/Physical Management Training) and DDS Medication Administration.
  • Ability to follow oral and written instructions.
  • Strong communication and negotiation skills.
  • Neat and accurate record keeping.
  • Experience with Microsoft products, use of email and Therap

  • Individuals in this job are likely to be required to perform lifting and transferring activities and may be required to perform a protective hold on program participants; may be exposed to risk of injury from abusive/assaultive participants.
