Job summary

Cypress Valley Center for Nursing & Rehabilitation in Reidsville, NC is seeking a PRN Certified Occupational Therapy Assistant (COTA) to join the rehabilitation team. Under the supervision of an Occupational Therapist, you will provide treatment interventions and support residents to regain independence.

This role emphasizes flexible scheduling, accurate documentation, collaboration with nursing staff and families, and occasional travel to nearby facilities to meet patient care needs.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Current state license or registration as a COTA.
• Experience in a skilled nursing, rehabilitation, or long-term care setting preferred but not required.
• Ability to safely assist residents with transfers and mobility, including lifting up to 50 pounds.

Responsibilities

• Implement occupational therapy treatment plans as directed by the supervising OT.
• Assist residents in improving functional mobility, ADLs, and independence.
• Accurately document treatment sessions, resident progress, and outcomes in accordance with facility policies.
• Collaborate with therapists, nursing staff, physicians, and family members to achieve the best possible resident outcomes.
• Educate residents, caregivers, and staff on therapy techniques, safety, and home exercise programs.
• Participate in interdisciplinary care planning and quality improvement initiatives.
• Travel to nearby affiliated facilities as needed to meet resident care needs.

Position Summary
