Continuum Rehab Group is seeking a Certified Occupational Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities in Prescott Valley, AZ.

Qualifications

• Licensed Occupational Therapy Assistant in good standing.
• Open to therapists at all experience levels.
• Proficient in skilled therapy techniques.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides skilled therapy services to patients with physical functioning disorders.
• Assesses and documents patient progress and communicates with supervising OT to adjust treatments.
• Provides consultation and counseling to patients and families related to physical disorders.
• Assists with discharge summaries per regulatory requirements.
