Job summary

Avamere Skilled Advisors, LLC is seeking a Certified Occupational Therapy Assistant (COTA) to provide high-quality patient care at Avamere Sunnyside in Salem, OR. The role operates PRN, under licensed therapist supervision, and emphasizes accurate documentation and collaboration with the care team.

The COTA will implement established treatment plans, communicate with patients and families, maintain licensure, and contribute to department operations while supporting overall patient outcomes.

Qualifications

• Graduate of an approved Occupational Therapy Assistant curriculum.
• Active CPR/BLS certification required.
• In good standing with regulatory/licensing boards.
• Knowledge of Medicare and payer sources.

Responsibilities

• Implements and executes treatment plans under supervision of a licensed Occupational Therapist.
• Communicates with patients, families, physicians, and team members.
• Documents written notes per department and insurance requirements.
• Maintains licensure and continuing education as required.
• Coordinates treatment goals with the rehab team and reports changes promptly.
• Prepares and submits patient charges accurately for billing.
• Attends required meetings and participates in department operations.
