Job summary

Continuum Rehab Group is seeking a Certified Occupational Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care facilities in Grants, NM. The role offers a sign-on bonus and a comprehensive benefits package.

Licensed OT Assistant required, with opportunities for mentorship, professional growth, and specialty certification paid by CRG. Open to therapists at all experience levels and committed to exemplary patient care.

Qualifications

• Licensed OT Assistant credential required.

• Experience in skilled nursing/long-term care settings preferred.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment to patients under supervision of an Occupational Therapist.
• Provides skilled therapy services to patients needing rehabilitation.
• Assesses patient progress and communicates with supervising OT to adjust treatments.
• Provides consultation to patients and families related to physical disorders.
• Assists with discharge planning and documentation as required.

Experience/Qualifications:

Licensed Occupational Therapy Assistant

This position is open to therapists at all experience levels.

Some duties for this position include:

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Occupational Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and document patient’s progress and communicates with supervising Occupational Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
