Job summary

Outcome HC's skilled nursing facility seeks a Certified Occupational Therapy Assistant (COTA) to join an interdisciplinary care team. The COTA will implement individualized treatment plans under an OT's supervision, enhance residents' independence, safety, and functional ability.

The role focuses on ADLs, mobility, cognitive interventions, and adaptive equipment training, with emphasis on accurate documentation and adherence to regulatory standards; prior SNF/geriatric experience is preferred

Qualifications

• Current state COTA license/certification.
• Strong clinical skills and attention to detail.
• Experience in a skilled nursing facility or geriatric setting preferred but not required.
• Ability to work independently and as part of a collaborative team.

Responsibilities

• Provide direct occupational therapy services under the supervision of a licensed OT in accordance with the resident's plan of care.
• Implement individualized treatment programs focused on functional improvement, ADLs, mobility, and safety.
• Deliver therapeutic interventions including exercises, ADL retraining, cognitive interventions, and adaptive equipment training.
• Instruct residents in use of assistive devices and adaptive equipment to maximize independence and safety.
• Document treatment sessions, resident progress, and outcomes according to regulatory and facility standards.
• Communicate resident progress and treatment outcomes with the supervising OT, residents, families, and caregivers.
• Provide education on safe mobility, fall prevention, and strategies to support independence in daily activities.
• Maintain accurate documentation for billing and reimbursement and adhere to facility policies.
