Job summary

Personal Touch Home Aides of New York is seeking a compassionate Certified Occupational Therapy Assistant (COTA) for home visits in Staten Island, NY. This full-time role requires an active NY COTA license and offers daytime and after-school case coverage to support patients in their homes.

You will deliver high-quality occupational therapy, document progress, and collaborate with a dedicated care team to promote independence and quality of life for diverse patient populations.

Qualifications

• Licensed/regulated as a COTA in NY state.
• Graduate of ACOTE-accredited OTA program.
• Strong verbal, written, and organizational skills.
• Experience with pediatric patients preferred.

Responsibilities

• Assess patient status and home environment for safety and independence.
• Execute treatment plans and communicate progress to physicians.
• Teach or retrain daily living activities as needed.
• Fabricate splints and train use of adaptive equipment.
• Train patients in prosthetic/orthotic device use.
• Maintain clinical records and reports per policy.
• Coordinate with care team for integrated plans.
• Attend case conferences and care planning meetings.
• Identify family needs for home health services.
• Educate family/caregivers on treatment regimes.
• Assist in discharge planning for patients.

JOB OVERVIEW:

The Certified Occupational Therapy Assistant (COTA) provides compassionate occupational therapy services that help patients improve independence and quality of life while working closely with the care team to deliver personalized, high-quality care.

RESPONSIBILITIES:

1. Assess patient’s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.) assess home environment for hazards or barriers to more independent living. Identifies equipment needs. Participates in the development of the total plan of care.
2. Execute treatment program and goals for improved function. Communicates plan of care to referring physician and other Organization professionals.
3. May teach new skills or retrain patient in once familiar daily activities that have been lost due to illness or injury.
4. May fabricate splints and instruct patient in the use of various types of adaptive equipment to improve function.
5. May train patient in the use of prosthetic and/or orthotic devices.
6. Maintains appropriate clinical records, clinical notes, summary reports and reports to the physician any changes in the patient’s condition. Submits these records in accordance with policy.
7. Maintains contact/communication with other personnel involved in the patient’s care to promote coordinated, efficient care.
8. Attends and contributes to in-services, case conferences and other meetings as required to ensure coordinated and comprehensive plans of care for the patients of the Organization.
9. Identifies patient and family/caregiver needs for other home health services and refers as appropriate.
10. Instructs patient’s family/caregiver and other health care personnel in patient’s treatment regime when indicated and appropriate.
11. Participates in discharge planning for patient.

QUALIFICATIONS:
