Job summary

Personal Touch Home Care in Staten Island, NY is seeking a Certified Occupational Therapy Assistant (COTA) to deliver compassionate occupational therapy services within a collaborative care team. The role focuses on improving patient independence and quality of life through personalized treatment plans.

Qualifications

  • Certified Occupational Therapy Assistant qualification and NY license requirement.
  • Graduate of ACOTE-accredited OTA program.
  • Pediatric experience or willingness to work with pediatric patients.

Responsibilities

  • Assess patient’s functional status and home environment to identify needs and plan care.
  • Execute treatment programs and communicate progress to the care team.
  • Instruct patients and families in daily living activities and use of adaptive devices.
  • Maintain clinical records and reports following policy.
  • Collaborate with other clinicians to ensure coordinated care.
  • Assist in discharge planning and coordination of home health services.

JOB OVERVIEW: The Certified Occupational Therapy Assistant (COTA) provides compassionate occupational therapy services that help patients improve independence and quality of life while working closely with the care team to deliver personalized, high-quality care.
RESPONSIBILITIES:
  • Assess patient’s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.) assess home environment for hazards or barriers to more independent living. Identifies equipment needs. Participates in the development of the total plan of care.
  • Execute treatment program and goals for improved function. Communicates plan of care to referring physician and other Organization professionals.
  • May teach new skills or retrain patient in once familiar daily activities that have been lost due to illness or injury.
  • May fabricate splints and instruct patient in the use of various types of adaptive equipment to improve function.
  • May train patient in the use of prosthetic and/or orthotic devices.
  • Maintains appropriate clinical records, clinical notes, summary reports and reports to the physician any changes in the patient’s condition. Submits these records in accordance with policy.
  • Maintains contact/communication with other personnel involved in the patient’s care to promote coordinated, efficient care.
  • Attends and contributes to in-services, case conferences and other meetings as required to ensure coordinated and comprehensive plans of care for the patients of the Organization.
  • Identifies patient and family/caregiver needs for other home health services and refers as appropriate.
  • Instructs patient’s family/caregiver and other health care personnel in patient’s treatment regime when indicated and appropriate.
  • Participates in discharge planning for patient.

QUALIFICATIONS:
