About the Role

Our Home Health COTAs are essential to providing quality care. You will support patients through direct care and collaborate with an interdisciplinary team to ensure excellent care experiences for both patients and their families.

Sequatche, Bledsoe, Grundy, and Marion Counties

Responsibilities

1. Perform ongoing evaluation of patients' functional status
2. Teach and retrain patients in daily activities
3. Instruct family/caregivers on treatment regimens
4. Follow treatment plans and goals as advised by the Registered Occupational Therapist
5. Maintain clinical records and documentation
6. Ensure compliance with legal and regulatory requirements
7. Perform additional duties as assigned

Qualifications

• Graduation from an ACOTE-accredited occupational therapy assistant program
• Current NBCOT certification and licensure in the state of practice
• At least 1 year of COTA experience
• Home health experience preferred
• Commitment to excellence in clinical practice and documentation
