Job summary

West Liberty Nursing & Rehab is hiring a Certified Occupational Therapy Assistant (COTA) to join our compassionate care team. You will deliver OT interventions under supervision, assist residents with ADLs and mobility, and monitor responses to treatment in a compliant, patient-centered environment.

The role requires an accredited OT Aide degree, current COTA licensure, and 1–2 years in geriatric or SNF settings. Familiarity with PDPM and SNF documentation is a plus.

Qualifications

• Associate’s degree from an accredited Occupational Therapy Assistant program.
• Current state licensure as a Certified Occupational Therapy Assistant (COTA).
• 1–2 years of experience in a skilled nursing facility or geriatric care setting preferred.
• Familiarity with PDPM and SNF documentation standards is a plus.

Responsibilities

• Deliver occupational therapy interventions as outlined in the treatment plan by the supervising OT.
• Assist residents with therapeutic exercises and ADLs to improve independence and mobility.
• Monitor resident responses and communicate progress or changes to the OT.
• Support the use of adaptive equipment and provide instruction to residents and caregivers.
• Maintain a safe and engaging therapeutic environment to promote participation.
• Participate in interdisciplinary care planning meetings for coordinated care.
• Document treatment sessions and progress in accordance with policies and regulations.
• Adhere to HIPAA, infection control, and OSHA standards.

Position Overview:

• · Participate in interdisciplinary team meetings and contribute to care planning discussions to ensure coordinated, resident-centered care
• · Accurately document treatment sessions, resident progress, and observations in accordance with facility policies and state and federal regulations.
• · Adhere to HIPAA guidelines, infection control protocols, and OSHA standards to ensure regulatory compliance and resident safety.

Education
