Job summary

Qualifications

  • Licensed/regulated as a COTA in NY state.
  • Graduate of an ACOTE-accredited Occupational Therapy Assistant program.
  • Strong verbal, written, and organizational skills.
  • Pediatric experience or willingness to work with pediatric patients required.

Responsibilities

  • Assess patient’s functional status and home environment; identify equipment needs and participate in the total plan of care.
  • Execute treatment program and goals; communicate plan of care to physician and team.
  • Teach new skills or retrain patients in daily activities.
  • Fabricate splints and instruct on adaptive equipment usage to improve function.
  • Train patients in use of prosthetic/orthotic devices as needed.
  • Maintain clinical records and reports; update physician changes.
  • Maintain communication with care team for coordinated care.
  • Attend in-services and case conferences for coordinated plans of care.
  • Identify needs for other home health services and refer as appropriate.
  • Instruct families/caregivers in treatment regime; assist in discharge planning.

JOB OVERVIEW:

The Certified Occupational Therapy Assistant (COTA) provides compassionate occupational therapy services that help patients improve independence and quality of life while working closely with the care team to deliver personalized, high-quality care.

RESPONSIBILITIES:
  • 1. Assess patient’s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.) assess home environment for hazards or barriers to more independent living. Identifies equipment needs. Participates in the development of the total plan of care.
  • 2. Execute treatment program and goals for improved function. Communicates plan of care to referring physician and other Organization professionals.
  • 3. May teach new skills or retrain patient in once familiar daily activities that have been lost due to illness or injury.
  • 4. May fabricate splints and instruct patient in the use of various types of adaptive equipment to improve function.
  • 5. May train patient in the use of prosthetic and/or orthotic devices.
  • 6. Maintains appropriate clinical records, clinical notes, summary reports and reports to the physician any changes in the patient’s condition. Submits these records in accordance with policy.
  • 7. Maintains contact/communication with other personnel involved in the patient’s care to promote coordinated, efficient care.
  • 8. Attends and contributes to in-services, case conferences and other meetings as required to ensure coordinated and comprehensive plans of care for the patients of the Organization.
  • 9. Identifies patient and family/caregiver needs for other home health services and refers as appropriate.
  • 10. Instructs patient’s family/caregiver and other health care personnel in patient’s treatment regime when indicated and appropriate.
  • 11. Participates in discharge planning for patient.
