Job summary

Personal Touch Home Care in Queens, NY, is seeking a full-time Certified Occupational Therapy Assistant (COTA) to provide compassionate OT services and improve patient independence. NY COTA license required; license validity and full-time commitment are essential.

Qualifications

• Licensed/certified as a COTA where required in NY.
• Graduate of an ACOTE-accredited OTA program.
• Strong verbal, written, and organizational skills.
• Pediatric experience or willingness to work with pediatric patients.

Responsibilities

• Assess patient functional status and home environment to identify equipment needs.
• Execute treatment plans and communicate with referring physician and other professionals.
• Instruct patient and family in daily activities and use of adaptive equipment.
• Fabricate splints and instruct on use of orthotic devices.
• Train patient in prosthetic devices and discharge planning.

JOB OVERVIEW

The Certified Occupational Therapy Assistant (COTA) provides compassionate occupational therapy services that help patients improve independence and quality of life while working closely with the care team to deliver personalized, high‑quality care.

RESPONSIBILITIES

• Assess patient’s functional status and home environment to identify equipment needs, and participate in developing the total plan of care.
• Execute treatment program and goals for improved function; communicate plan of care to referring physician and other professionals.
• Teach new skills or retrain patient in familiar daily activities that have been lost due to illness or injury.
• Fabricate splints and instruct patient in the use of various types of adaptive equipment to improve function.
• Train patient in the use of prosthetic and/or orthotic devices.
• Maintain appropriate clinical records and reports to the physician any changes in the patient’s condition; submit these records in accordance with policy.
• Maintain contact and communication with other personnel involved in the patient’s care to promote coordinated, efficient care.
• Attend and contribute to in‑services, case conferences, and other meetings as required to ensure coordinated and comprehensive plans of care for the patients of the organization.
• Identify patient and family/caregiver needs for other home health services and refer as appropriate.
• Instruct patient’s family/caregiver and other health care personnel in patient’s treatment regime when indicated and appropriate.
• Participate in discharge planning for patient.
