Job Summary

Responsible for an assigned group of patients, typically geographically related, and accountable for the oversight and management of occupational therapy services in the home setting under the supervision of a licensed Occupational Therapist (OT). Implements occupational therapy patient care programs intermittently in the home and reports changes in the patient's condition, performance, and response to treatment to the OT, following physician’s orders.

Essential Functions
  • Manage a daily caseload, ensuring timely care and documentation to meet patient needs as prescribed in the care plan and physician's orders.
  • Gather relevant health data from patient assessments, telephonic contacts, and electronic medical records.
  • Implement patient care programs, including modalities, procedures, exercise programs, and patient education as directed by the OT.
  • Document and execute a care plan tailored to patient and family needs, including discharge planning and resource utilization.
  • Use clinical reasoning to conduct assessments, evaluate therapy effectiveness, and prioritize patient needs.
  • Provide patient and family education on deficits, progress, and therapeutic techniques, including home program instructions and equipment use.
  • Maintain communication with the multidisciplinary team, updating on patient status and needs.
  • Integrate goals from the care plan into therapy sessions, documented in visit notes.
  • Coordinate with physicians and therapy leadership to ensure timely feedback and adjustments to the care plan.
  • Perform treatments and procedures in compliance with state laws, healthcare standards, and agency policies.
  • Support shared governance and research initiatives to improve clinical outcomes.
Physical Requirements

Requires walking, standing, sitting, lifting (up to 50 pounds), reaching, stooping, bending, pushing, and pulling. Must be able to travel extensively, work in various home environments, and communicate effectively in English. Must have good sensory perception, finger dexterity, critical thinking, and the ability to respond quickly to changing conditions. Flexible schedule required. Use of PPE may be necessary due to exposure risks.
