Job summary

Fallsview Rehabilitation & Nursing is seeking a Full-time Occupational Therapist Assistant (OTA) in New Jersey. PRN also available. The OTA will assist the Occupational Therapist in resident assessments, implement ADL training, and participate in interdisciplinary care with PTs and SLPs.

Strong documentation, EMR proficiency, and NJ licensure are required. Remote monitoring and ongoing education for residents and families are integral parts of the role.

Qualifications

• Must hold a New Jersey OTA license.
• Experience with resident assessments and ADL training.
• Strong documentation and EMR experience.
• Ability to work in a team with PTs and SLPs.

Responsibilities

• Assist the Occupational Therapist in assessing residents and gathering objective data.
• Implement intervention plans and ADL training under OT guidance.
• Monitor residents' performance and report changes to supervising PT.
• Maintain accurate, timely documentation in EMRs (Net Health/PCC) and ROS notes.
• Educate residents, families, and care staff on safe mobility techniques.
• Conduct remote check-ins through remote therapeutic monitoring (RTM).
• Collaborate with rehab staff to provide comprehensive care.

Responsibilities:

• Assist the Occupational Therapist in assessing residents
• functional abilities and gathering objective data for collection during Annual, Quarterly, Falls, Significant Change assessments.
• Treatment Support: Implement intervention plans, including ADL training, based on the guidance of the Occupational Therapist.
• Monitoring: Observe residents' performance and report any changes or concerns to the supervising Physical Therapist.
• Documentation: Maintain accurate and timely documentation of resident progress and any interventions provided. Comfortable and knowledgeable with Point of Service(PoS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs (Net Health/PCC).
• Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring (RTM) with discharged patients to home or next level of care.
• Interdisciplinary Collaboration: Collaborate with other rehab staff, including Physical Therapists and Speech-Language Pathologists, to provide comprehensive care.
• Must have state license in New Jersey
