Discovery at Home is seeking a certified Occupational Therapist Assistant (COTA) in Florida to support evaluation and development of rehabilitation plans under the supervision of an Occupational Therapist. You will educate patients and caregivers, document progress, and collaborate with a care team to ensure comprehensive treatment and improved independence.

Qualifications

• Must have a COTA degree and an active COTA license in the applicable state.
• One year of home health experience preferred.
• Knowledge of Medicare and Medicaid guidelines required.
• Strong verbal and written communication skills essential.
• Valid driver’s license and reliable transportation.

Responsibilities

• Under the direction of the Occupational Therapist, assist in evaluating and developing the rehabilitative plan of care and in re-evaluations.
• Educate patients and caregivers on techniques to improve mobility, reduce pain, and prevent injuries.
• Maintain accurate and timely documentation and progress notes.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Guide patients in prescribed therapeutic self-care and activities to improve independence.
• Attend and participate in department staff meetings and case conferences.

• Educate patients and caregivers on proper techniques to improve mobility, reduce pain, and prevent injuries
• Maintain accurate and timely documentation and progress notes.
• Collaborate with other healthcare professionals to ensure patients receive comprehensive care
• Guide and instructs patients in prescribed therapeutic self-care and creative activities that are directed toward improving independence and functioning.
• Attend and participate in department staff meetings and case conference meetings.

Qualifications:

• COTA degree from an accredited program
• A COTA license from applicable state
• One year of home health experience required
• Must have knowledge of Medicare and Medicaid guidelines.
• Current CPR certification
• Valid driver’s license and reliable transportation
• Excellent verbal and written communication skills
• Basic computer skill proficiency. Ability to use Microsoft Windows.
