Job Overview

This role involves planning, implementing, coordinating, monitoring, and evaluating patient care activities. The candidate should possess clinical and/or functional knowledge relevant to the position, either directly or through delegation. The incumbent reports to the Administrative Team and collaborates with Clinical Associates to meet patient needs, utilizing effective communication and available resources to promote quality, cost-effective care.

Key Responsibilities

• Document progress notes and treatments according to departmental policies across all practice settings.
• Deliver treatments following clinical protocols to ensure patient satisfaction and identify barriers to improve care processes.
• Implement optimal patient care within assigned caseload efficiently and cost-effectively; suggest improvements and demonstrate flexibility in scheduling.
• Accurately document charges and treatments, including equipment use, in a timely manner.
• Communicate effectively with healthcare team members, patients, families, and physicians; identify and address communication barriers.
• Develop and instruct patients on safe exercises and provide written home exercise instructions, ensuring patient understanding.
• Participate in educational activities, contribute to co-worker and community education, and develop educational materials.
• Assist in preparing patients and families for discharge, ensuring resources are in place, and attend patient case conferences.
• Perform clinical skills professionally, contribute to program development, and assist with departmental cleanliness and environment of care.
• Attend required meetings.

Minimum Requirements
