Job summary

VitalCaring Group seeks a Certified Occupational Therapy Assistant (COTA) to join our home health team. You will provide one-on-one, goal-oriented therapy in patients' homes under the supervision of a Licensed OT, focusing on restoring independence in daily living activities.

You’ll work autonomously in a field-based setting, with strong emphasis on patient safety, care plan adherence, and caregiver education.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Active COTA license/certification in the state of practice.
• Home Health experience strongly preferred.
• Current CPR certification.
• Ability to work independently in a field-based environment.
• Strong clinical observation and communication skills.

Responsibilities

• Deliver personalized, goal-driven home health therapy under OT supervision.
• Provide hands-on therapy to improve ADLs, mobility, strength, and coordination.
• Retrain patients in dressing, bathing, meal prep, and self-care tasks.
• Assess progress and report function and safety in the EMR.
• Educate patients and caregivers on exercises, safety strategies, and home adaptations.
• Communicate changes in condition to OT and care team.
• Documentation in the EMR in a timely and accurate manner.

What You’ll Do

Work under the direction of a Licensed Occupational Therapist to deliver personalized, goal-driven care:

• Implement treatment plans to improve patients’ ability to perform daily activities
• Provide hands‑on therapy focused on strength, coordination, mobility, and self‑care
• Retrain patients in ADLs (dressing, bathing, meal prep, etc.)
• Assess and report on patient progress, function, and home safety needs
• Educate patients and caregivers on techniques, exercises, and safety strategies
• Identify changes in condition and communicate proactively with the OT and care team
• Complete timely, accurate documentation in the EMR
• Support a safe, effective, and coordinated plan of care

What We’re Looking For

We’re seeking COTAs who understand the independence and clinical awareness required in home health:

• Graduate of an accredited Occupational Therapy Assistant program
• Active COTA license/certification in the state of practice
• Home Health experience strongly preferred
• Current CPR certification
• Ability to work independently in a field-based environment
• Strong clinical observation, communication, and time management skills
• Valid driver’s license, reliable transportation, and auto insurance
