StoneBridge Senior Living is seeking a compassionate Certified Nursing Assistant to support nurses and care for elderly residents. Responsibilities include assisting with daily living activities, mobility, dining service coordination, and documentation.

Qualifications

• Certified Nursing Aide in good standing required.
• High School Diploma or GED preferred.

Responsibilities

• Provides supportive services to nurse(s) and other staff and performs duties as assigned.
• Assists residents with activities of daily living per care plans and policies.
• Assists with lifting, turning, positioning, and transport of residents.
• Coordinates dining room services, meal delivery, feeding assistance, and documents intake.
• Delivers nutritional supplements and tracks intake; reports weight changes to nurse.
• Assists with skin checks and reports issues to nurse/supervisor.
• Supports toileting program activities and documents progress.

A Certified Nursing Assistant is responsible for supporting medical staff in administering care to residents with various injuries, illnesses or disabilities. The position requires patience, compassion, and a desire to care for the elderly in a gentle and empathetic manner.

Responsibilities and Duties CNA

• Provides supportive services to nurse(s) and other staff as needed and performs duties as assigned.
• Assists resident with or performs activities of daily living for resident in accordance with care plans and established policies and procedures.
• Assists resident with lifting, turning, moving, positioning, and transporting into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
• Coordinates dining room services at assigned mealtimes, including set-up and clean-up, meal tray delivery, feeding assistance, and documentation of meal intake.
• Delivers nutritional supplements to residents at assigned times and provides assistance as necessary to ensure intake. Documents intake accordingly.
• Assists with weighing residents according to facility policy, and records weight in designated locations. Reports weight changes to nurse and supervisor.
• Assists with tracking the condition of the resident’s skin. Reports any presence of pressure areas, skin breakdown or skin tears to nurse and supervisor.
• Assists nursing staff in carrying out toileting program activities.
• Completes flow sheets daily to indicate that the specified task was done. Documents the progress of each resident as directed by supervisor.
• Keeps nurse and supervisor informed of factors that interfere with being able to perform the work as assigned (i.e., resident refusal, reassigned to nursing unit).
• Reports changes in a resident’s condition to the resident’s nurse and supervisor.

Responsibilities and Duties Central Supply

• Inventory Management: Conduct regular weekly or monthly counts to maintain correct par levels (the minimum stock needed).
• Ordering and Receiving: Order medical and facility supplies, receive shipments, and inspect items for damage.
• Distribution: Deliver supplies to nursing stations and departments, keeping stock rooms clean and organized.

Requirements

Qualifications and Skills

• Certified Nursing Aide in good standing with the certification board or is a graduate of a state-approved nursing assistant training program and passed a state-approved competency examination (written and skills) within 120 days of employment REQUIRED
• High School Graduate/GED preferred.
• Current CPR/BLS certification preferred.
• At least one year of experience as a certified nursing assistant in long-term care preferred.
• Must be able to express self adequately in written and oral communication and to communicate effectively in an interdisciplinary care setting with residents, families, and staff members.
