Job summary

StoneBridge Senior Living is seeking a Certified Nursing Assistant to support nursing staff and provide compassionate care for residents with injuries, illnesses, or disabilities. The role emphasizes daily living support, feeding assistance, and care coordination in a senior living environment.

The successful candidate will have CNA certification, preferably CPR/BLS, and experience in long-term care, with strong written and verbal communication within a multidisciplinary team.

Qualifications

• Certified Nursing Assistant in good standing or graduate of a state-approved nursing assistant training program who passed a state competency exam within 120 days of employment.
• High School Diploma or GED preferred.
• Current CPR/BLS certification preferred.
• At least one year of experience as a CNA in long-term care preferred.
• Ability to communicate effectively in written and oral form within an interdisciplinary care setting.

Responsibilities

• Provides supportive services to nurse(s) and other staff as needed and performs duties as assigned.
• Assists residents with activities of daily living per care plans and established policies.
• Assists residents with lifting, turning, moving, positioning, and transporting.
• Coordinates dining room services at meals, including setup, delivery, feeding and meal intake documentation.
• Delivers nutritional supplements and documents intake.
• Assists with weighing residents and reports weight changes to nurse and supervisor.
• Monitors skin condition and reports any pressure areas or skin issues.
• Aids with toileting program activities and documents progress.
• Keeps nurse and supervisor informed of factors interfering with work and reports changes in resident condition.

Responsibilities And Duties

• Provides supportive services to nurse(s) and other staff as needed and performs duties as assigned.
• Assists resident with or performs activities of daily living for resident in accordance with care plans and established policies and procedures.
• Assists resident with lifting, turning, moving, positioning, and transporting into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
• Coordinates dining room services at assigned mealtimes, including set-up and clean-up, meal tray delivery, feeding assistance, and documentation of meal intake.
• Delivers nutritional supplements to residents at assigned times and provides assistance as necessary to ensure intake. Documents intake accordingly.
• Assists with weighing residents according to facility policy, and records weight in designated locations. Reports weight changes to nurse and supervisor.
• Assists with tracking the condition of the resident’s skin. Reports any presence of pressure areas, skin breakdown or skin tears to nurse and supervisor.
• Assists nursing staff in carrying out toileting program activities.
• Completes flow sheets daily to indicate that the specified task was done. Documents the progress of each resident as directed by supervisor.
• Keeps nurse and supervisor informed of factors that interfere with being able to perform the work as assigned (i.e., resident refusal, reassigned to nursing unit).
• Reports changes in a resident’s condition to the resident’s nurse and supervisor.

Requirements

Qualifications And Skills

• Certified Nursing Aide in good standing with the certification board or is a graduate of a state-approved nursing assistant training program and passed a state-approved competency examination (written and skills) within 120 days of employment REQUIRED
• High School Graduate/GED preferred.
• Current CPR/BLS certification preferred.
• At least one year of experience as a certified nursing assistant in long-term care preferred.
• Must be able to express self adequately in written and oral communication and to communicate effectively in an interdisciplinary care setting with residents, families, and staff members.
