St. Ann's Community Offers:

  1. Full time Day & Evening Shifts
  2. Starting rate - $18.50/hr.
  3. $4/hr. Evening/Night Differential
  4. $6/hr. All Weekend Shifts Differential

Job Description:

  1. Provide high quality, individualized patient care in collaboration with our nursing team.
  2. Take initiative while assisting and maintaining patient care.
  3. Assist the resident in activities necessary as per individualized care planning.
  4. Transport Elders to and from activities.
  5. Perform evaluation and reporting.
  6. Provide resident and family with emotional support individualized care planning.
  7. Receive/give reports to team members/team leaders to appropriate professional staff.

Candidate Requirements:

  1. Valid NYS Certified Nursing Assistant License (CNA) - Required
  2. Minimum of 1 year experience working with Elders - Preferred
  3. Ability to work in a team-oriented work group.
  4. Proficient in various computer programs and Medical Records Documentation system.
