Job summary

Symbii Afton Home Health & Hospice is seeking a Home Health Aide to provide compassionate in-home care. Responsibilities include assisting with personal hygiene, meals, and feeding; monitoring vital signs; aiding mobility; and ensuring a safe, clean living environment.

You will document care and report changes to the supervising nurse. Requirements include at least 18, CPR certification, a valid driver’s license, and reliable insured transportation.

Qualifications

• Meets the training requirements per State and Federal laws.
• At least 18 years of age.
• Ability to read and follow written instructions and document care given.
• Self-directing with ability to work with little supervision.
• Empathy for the needs of the ill, injured, frail and the impaired.
• Possess and maintains CPR certification.
• Licensed driver with automobile insured and in good working order.

Responsibilities

• Provide personal care: baths, back rubs, oral hygiene, shampoos, changing bed linen, dressing and undressing, skin care.
• Plan and prepare nutritious meals and assist in feeding if necessary.
• Take and record temperatures, pulse, respiration, and blood pressure as ordered.
• Assist in ambulation and exercise per care plan.
• Perform routine procedures within extensional therapy as ordered with competency.
• Support patient self-administration of medications as directed.
• Handle laundry and keep patient living area clean and safe.
• Report patient condition changes to the assigned nurse and document accordingly.
• Adhere to organization care procedures and professional conduct standards.

DUTIES & RESPONSIBILITIES

Responsibilities of the home health aide include, but are not limited to, the following:

• Providing personal care including:A. BathsB. Back rubsC. Oral hygieneD. Shampoos E. Changing bed linenF. Assisting patients with dressing and undressingG. Skin care to prevent breakdownH. Assisting the patient with toileting activitiesI. Keeping patient’s living area clean and orderly, as appropriate
• Planning and preparing nutritious meals.
• Assisting in feeding the patient, if necessary.
• Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency).
• Assisting in ambulation and exercise according to the plan of care.
• Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency).
• Assisting patient in the self-administration of medication.
• Doing patient’s laundry, as appropriate.
• Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.).
• Reporting on patient’s condition and significant changes to the assigned nurse.
• Adhering to the Organization’s documentation and care procedures and standards of personal and professional conduct.
