Job Summary

The hospice aide is a paraprofessional member of the interdisciplinary team who works under the supervision of a registered nurse. They perform various services to meet the patient's personal needs and promote comfort. Responsibilities include observing, reporting, and documenting patient care, ensuring quality, continuity, and safety in care delivery.

Duties & Responsibilities
  • Perform personal care tasks promptly as assigned; report inability to perform tasks to the Case Manager.
  • Ensure safety by properly using equipment such as foot stools, side rails, and oxygen.
  • Provide personal care including baths, back rubs, oral hygiene, shampoos, and changing bed linens.
  • Assist with dressing and undressing patients.
  • Plan and prepare nutritious meals, including shopping as assigned.
  • Assist with feeding and take/record vital signs when ordered.
  • Monitor skin condition to prevent breakdown over bony prominences.
  • Report patient condition and significant changes to the Case Manager.
  • Assist with ambulation and exercises as instructed.
  • Help with bedpans, urinals, light laundry, and perform simple procedures as ordered.
  • Provide respite care for patients, families, and caregivers as appropriate.
  • Maintain a clean and orderly living environment for the patient.
  • Adhere to documentation, care procedures, and professional standards.
  • Participate in quality improvement activities.
  • Respect patient and family/caregiver environment and needs.
  • Note: The aide does not administer medications or perform procedures requiring nursing licensure.
Job Requirements
  • Completion of a state-approved training program and competency evaluation, or licensure.
  • At least 18 years old.
  • Ability to read and follow instructions and document care.
  • Understanding of hospice philosophy and comfort providing care to terminally ill.
  • Good references, self-directed, with empathy and tact.
  • Current CPR certification.
  • Valid driver’s license, automobile insurance, and access to transportation.
  • Good communication and organizational skills.
