Job summary

RWJBarnabas Health is seeking a Certified Nursing Assistant to support the Long Term Care team in Toms River. You will assist with daily living activities, monitor vital signs, and document patient information in the EHR while maintaining confidentiality.

You will work evenings, providing hands-on care, collaborating closely with RNs and therapists to ensure safe, dignified patient experiences and continuity of care throughout admission to discharge.

Qualifications

• Current Certified Nursing Assistant in New Jersey.
• Experience assisting with activities of daily living is preferred.
• Ability to monitor vital signs and report changes promptly.

Responsibilities

• Assist patients with activities of daily living and basic nursing care.
• Take and record vital signs (BP, pulse, respiration, temperature).
• Assist with bedside care like bathing, mouth care and skin care.
• Document patient information accurately in EHR.
• Follow infection control protocols to keep patients and staff safe.
• Communicate effectively with patients, families, and the care team.
• Provide CPR/BLS during emergencies as trained.
• Support care plans from admission to discharge with teamwork.

As a Certified Nursing Assistant, a typical day may include:

• Assisting patients with activities of daily living and providing nursing care to patients including taking vital signs, , assisting with bedside care, i.e. bathing, mouth care, skin care.
• Monitor vital signs including blood pressure, pulse, respiration, and temperature to track patient health status
• Document patient information accurately in electronic health records (EHR) systems while maintaining strict confidentiality
• Implement infection control protocols to ensure a safe environment for patients and staff, adhering to facility policies
• Communicate effectively with patients, families, and healthcare providers to coordinate care plans and address concerns
• Support emergency response efforts by applying Basic Life Support (BLS) skills such as CPR when needed in acute situations
• Providing patient care, based on physical, education, safety, and related criteria, appropriate to the age of the patients
• Providing for effective and safe patient outcomes through practice, judgment and critical actions
• Ongoing communication with patients and nursing colleagues regarding status of patient’s care
• Participating in collaboration of our patient's plan of care from admission to discharge to ensure safe transitioning
