Responsibilities
- The provision of hands-on personal care.
- The performance of simple procedures as an extension of therapy or nursing services.
- Assistance in ambulation or exercises according to the plan of care.
- Assistance in administering medications ordinarily self-administered.
- Recording pertinent information pertaining to the clients' care and strict adherence to the HHA care plan on the clients' charts.
- Providing hands-on personal care including baths, oral hygiene, shampoos, assisting with/changing bed linens, dressing/undressing, skin care, toileting, keeping the living area clean and orderly.
- Planning and preparing light meals/snacks according to dietary requirements, and feeding the client if necessary.
- Performing range of motion and other simple procedures as ordered, with demonstrated competency.
- Meeting safety needs and using equipment safely and properly.
- Participating in an on-call rotation for holiday and weekend coverage.
- Taking vital signs, recording input/output as per care plan, with demonstrated competency.
- Observing and reporting patient condition changes to the Care Manager/clinician or Supervisor.
- Adhering to documentation, care procedures, and professional conduct standards.
- Maintaining timely communication and promptly returning calls.
- Participating in educational programs to meet regulatory guidelines.
- Exemplifying excellent customer service and professionalism.
- Performing other duties as assigned.
Qualifications
- Complete required training and competency evaluation as per regulations.
- CNAs must have certification.
- At least 18 years old with a high school diploma or equivalent.
- Ability to read, write, follow instructions, and document care.
- Self-directed with minimal supervision.
- Empathy for patients' needs.
- Related work experience preferred.
- Physically capable of performing duties.
- Maintain confidentiality and professional behavior.
- Current CPR certification.
- Licensed driver with insured, in-good-condition vehicle.
Job Limitations
The HHA will not perform nursing duties such as medication administration or procedures requiring nursing training, as per state Nurse Practice Act.
Note: Employment requires passing a drug screen, including testing for medical and recreational marijuana.
