Job summary

Senior Community Care of Northern Kentucky PACE is seeking a Certified Nursing Assistant to support participants in Florence, KY. This role focuses on daily living activities, health safety, and compassionate care in home and Day Center settings.

You will deliver restorative and supportive services, monitor health status, and collaborate with the Home Care Coordinator to ensure high-quality, person-centered care for older adults in our PACE program.

Qualifications

• Must have active CPR certification.
• Must have current STNA license.
• At least 1 year of experience with frail or elderly population.
• High School diploma or equivalent.
• Medical clearance for communicable diseases and up-to-date immunizations.
• Valid driver’s license, insurance, and transportation to homes.

Responsibilities

• Provide activities of daily living and restorative care.
• Monitor health status and maintain safe home environment.
• Travel to participant homes and Day Center as needed.
• Assist with meals, hygiene, mobility, and medications as applicable.
• Work with Home Care Coordinator to coordinate visits and plans of care.

About the job:

Pay: $19.40 - $24.50 (based on experience)

Schedule: M-F, 4 Days per week - 10 hour shifts , Occasional weekends and evenings required.

Counties that we also travel to: Grant, Gallatin, Carrollton county areas.

The Home Care CNAis responsible for providing and assisting participants with activities of daily living and restorative/supportive care necessary to ensure the physical and health safety of the home environment. Assists in monitoring and reporting the health status and providing a safe environment for the PACE participants of Senior Community Care of State (SCC). As assigned, works shifts in the SCC Day Center and in participant homes and provides care and services to participants and assists with the provision of a safe and effective day program.

Required Qualifications:To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

• Must have an active and up-to-date certification in CPR.
• Must have - Current STNA license
• A minimum of one-year experience in working with the frail or elderly population.
• High School diploma or equivalent required.
• Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
• Must have a valid driver’s license, proof of insurance and have means of transportation. You will be traveling to participant homes.

Preferred:

• Bilingual skills are a plus!
• Consults each day with the Home Care Coordinator (or designee) to be updated on changes in participant status, changes in plan of care, and to confirm scheduled visits.
• Assists participants in the home with the following duties:
  • Personal hygiene such as skin, mouth, hair care and bathing.
  • Assists participants in and out of bed assisting with ambulation.
  • Assisting with prescribed exercise which participants and aide have been taught by appropriate personnel.
  • Prepares meals, including therapeutic diets, assists participants with meals.
  • Assist participants to the bathroom or in using commodes, bedpans, or urinals.
  • Assisting participants with medications which are ordinarily self-administered.
  • Other duties taught by a health profession for a specific participant such as: taking of vitals, changing colostomy bags, changing of non-sterile dressing and non-sterile bowel and bladder hygiene care.
• Performs directed household / homemaker services.
• Performs the following duties, as assigned or needed, in the Day Center:
  • Assists participants and or supervises with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting) in the Day Center as assigned or needed.
  • Monitors and assists participants with meals at feeding table.
  • Assists, throughout the day, in maintaining clean and orderly day rooms, kitchen and personal care areas.
  • Performs and or assists with laundry, including clothes for participants.
  • Assists the Drivers with the loading and unloading of participants from vehicles.
