What You’ll Do:
  • Provide personal care including bathing, dressing, grooming, and toileting for hospice and home health care patients in their homes or facilities

  • Offer emotional support to patients and their families during end-of-life or recovery care

  • Assist with mobility and transfers, ensuring patient safety and comfort

  • Document daily care activities accurately and timely

  • Communicate changes in patient condition to supervising nurse

  • Support the team in maintaining a calm, comforting, and dignified environment

What We’re Looking For:
  • Active CNA certification in the state of Michigan

  • Previous experience in hospice or home health care preferred, but not required

  • Strong interpersonal and communication skills

  • Reliable transportation, valid driver’s license, and current auto insurance

  • Willingness to travel within Wayne, Oakland, and Macomb Counties
