Qualifications

Responsibilities

• Provide personal care including baths, oral hygiene, and changing bed linens.
• Assess patient condition and report changes to Case Manager.
• Assist with feeding, medications reminders, and meal preparation.
• Maintain patient safety and use equipment properly.
• Provide respite for family/caregiver when on-site.
