About Position: The primary responsibility of the Certified Nursing Assistant is to provide hands-on services that assist individuals, their family or both with home management tasks, personal care tasks, or supervision of the client's activities to enable the individual, their family, or both to remain and function at home.   • Work Location: Pittsboro Area

Serving various counties: Chatham, Lee, Orange

  • Schedule: Part-Time/ PRN
  • Options for flexible scheduling
  • Does require working a minimum of one weekend a month (No Remote Option)
  • Required Travel: Yes, This positions requires frequent travel to patients homes.

Patient Population: Adult Geriatric

Duties/ Responsibilities –

  • As assigned by the Registered Nurse in the Nurse Aide Plan of Care, provide basic nursing care to assigned clients within their home, including tasks such as, but not limited to: mobility, bathing, dressing, hygiene, toileting, skin care, housekeeping and meal preparation.
  • Support care provided in rehabilitative therapies as directed by licensed therapists (OT, PT and ST) in the home environment.
  • Demonstrates safe transfer techniques and ambulation assistance.
  • Performs vita sign check and other support as delegated within the Nurse Aide Plan of Care.
  • Communicates effectively with clients, families, peers, agencies and other departments within PHS.
  • Documents tasks, time, observations, services, and client activities and submits according to policy.
  • Communicates any changes seen in client's condition or home environment to RN supervisor.
  • Abides by all established PHS policies, rules and regulations.

Qualifications –

Education: : High school diploma or equivalent; Successful completion of a state approved Nurse Aide I training and competency evaluation program. Minimum of one year of experience with frail or elderly clients and in-home care preferred.

  • Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment.
  • Ability to effectively interact with clients, families and Piedmont Health team.
  • Ability to read and understand short instructions, correspondence and communications.
  • Ability to effectively communicate through written and verbal language.
  • Ability to provide help and services to people of all ages, including older adults.
  • Must meet a standardized set of competencies for the specific position description established by Piedmont Health Senior Care and approved by Centers for Medicare and Medicaid Services prior to working independently.

Preferred

Current/valid License:

  • Certified Nursing Assistant I
  • Valid NC driver's license
  • CPR and First Aid, upon employment

Immunizations: Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment.

Pay Range: $17.30- $23.26
